What you need before you book

Please have the following ready before you make a booking for your procedure:

  1. Referral letter.
  2. Private health insurance details.

Additionally please inform the booking staff if any of the following apply to you:

  • You have been IDDM (Insulin-Dependent Diabetes Mellitus).
  • You have an implantable defibrillator.
  • You suffer with severe cardiac or lung disease.
  • If you are taking Warfarin, Clopidogrel (Plavix), Dabigatran (Pradaxa) or any other blood thinning medication.
  • You are wheelchair bound or have mobility difficulties.

Patient admission information

What you need after you book

  • Please check the Allevia Endoscopy Admission Form, sign it and advise the doctor of the medications you are currently taking.
  • If you have private health insurance, please bring a copy of your pre-approval insurance letter and applicable excess payments.
  • If you do not have medical insurance, we will calculate an estimated total cost of your procedure, which will be charged on admission.
  • On completion of your stay your account will be processed and the amount adjusted accordingly. The total amount charged varies from patient to patient and any extra charges will be expected to be settled on discharge.
  • Free wi-fi services are available throughout the hospital. Passwords are available from Allevia Endoscopy reception.
  • Please ensure you have arranged transport.
